NCT04742777 is a Phase 2 substudy from the University of Texas Health Science Center at San Antonio testing mTOR inhibition and other metabolism-modulating interventions in older adults, with immune, cognitive and functional endpoints. This substudy uses cardiac MRI with late gadolinium enhancement in 20 participants. Started February 2022. Active, not recruiting, primary completion 16 October 2026.

This work sits in the same research program as the PEARL and other rapamycin aging trials at UT Health San Antonio, where Matt Kaeberlein's PEARL trial separately tested rapamycin dosing in healthy adults.

The substudy is small and imaging-focused. It cannot move rapamycin's evidence grade for longevity outcomes on its own. Rapamycin stays at evidence grade B: strong mouse lifespan data and growing but small human trials.
